(A) Based on its commitment to the protection of coral reefs and their marine life, ReefKeeper International, Inc., a tax-exempt non-profit conservation organization, agrees to provide the material and services support listed below to (name of group), a non-profit conservation organization, to help (name of group) carry out a program of coral reef monitoring. ReefKeeper International will provide said material and services support specifically to enable (name of group), acting as a ReefKeeper ReefMonitor Affiliate, to conduct quarterly coral reef monitoring, based on the attached ReefKeeper point-line intercept survey method, of the following (number) coral reef sites: Site name Location (1) (2) (3) (4) (5) (B) Based on the conditions stated above, ReefKeeper International agrees to provide to (name of group), a ReefKeeper ReefMonitor Affiliate, the following materials and services: (B1) a monitoring start-up kit, to be delivered to the ReefMonitor Affiliate group within 30 days of the date of this Cooperative Agreement. The monitoring start-up kit will include at least 1 fifty-meter survey reel, 2 underwater clipboards, 1 coral species identification book, 1 box of plastic pencils, 1 package of clipboard clips, 20 sets of preprinted underwater data sheets, 1 x-ray-proof film transport bag, 4 rolls of 35 mm film, 2 blank videocassettes (8mm or vhs), and other assorted supplies, (B2) a quarterly credit of $75 U.S. per site monitored during the previous three months, which will be released as either cash or requested in-kind materials, upon receipt by ReefKeeper of completed data sheet copies and exposed film rolls corresponding to the sites monitored during the previous quarter. The credit may be applied to the procurement of photographic film and services, videocassettes, underwater data sheets, miscellaneous monitoring related supplies, or monitoring related equipment such as survey reels, clipboards, compasses, GPS units, and species identification guides, (B3) as part of the above cash/in-kind credit, one set of 3-inch-by-5-inch photo prints and a set of film negatives from each monitor site photo roll submitted to ReefKeeper, (B4) technical assistance to recruit, train and coordinate reef monitor volunteers, (B5) a donor recognition program to attract and reward donors of monitor related services and supplies (such as boat, photolab, and dive shop services), (B6) professional scientific data analysis support, (B7) writing and editorial preparation of narrative quarterly updates based on monitoring data and other pertinent information submitted to ReefKeeper, (B8) submission of formatted data and narrative quarterly updates to the Global Coral Reef Monitoring Network (Australian Institute of Marine Science) and to ReefBase (International Center for Living Aquatic Resources Management), (B9) printed publication of quarterly updates noted above, to be published as sections of ReefKeeper's quarterly ReefMonitor Report, (B10) 100 reprints of each quarterly ReefMonitor Report or relevant update sections, (B11) electronic publication of quarterly updates noted above in ReefKeeper's World Wide Web site (www.reefkeeper.org), and website archiving of all submitted monitor data, (B12) a web "page" in ReefKeeper's web site for the ReefMonitor Affiliate group, and a "hot link" to the group's own website if the group also has its own web site, and (B13) equal name and logo recognition as a ReefMonitor Affiliate group and full source credit on all printed and electronic publications of the group's monitoring data and updates. (C) This Coral Reef Monitoring Cooperative Agreement may be canceled by ReefKeeper International, Inc. or by (name of group) with 30 days notice provided by either mail, fax, or e-mail. Should this Cooperative Agreement be so canceled, (name of group) may keep all equipment, materials and supplies provided prior to the date of cancellation. In addition, ReefKeeper will honor any pending cash/in-kind credit requests as long as they are received by ReefKeeper no later than 10 days after cancellation of this Agreement and meet the conditions under paragraph B2 above.
